Default Adding a father's last name to the birth certificate
I had a baby with a guy and we weren't together when I delivery so I had to use my last name for the baby, now we have agreed to add his last last name to the baby and also added his name to the birth certificate, my question is how would i do that in the state of florida. also my litle girl in now 16 months old.

You can establish paternity by affidavit, then apply to amend the birth record.
