Step-father Adopting Daughter In Arizona

I am wondering how to go about having my husband adopt my daughter. The biological dad has never been on the birth certificate and I have never received any type of child support payment she is now 5-years old. Her Step-father has been helping me raise her since she was about 1-years old and we have now been married for 4-years now. What are my options and how do I go about doing this? We have never been able to find her biological dad?

No, everytime we thought he was found he would take off again. So, he hasn't set any visitation or received a request to pay support.
