Default The last name for a baby's birth certficate
I gave birth to a baby girl on Sunday morning. Monday i completed her birth cert. but WITHOUT her fathers name on it. I gave her my last name. Now i'm regretting that, i want her to have a different last name. If i've already finished the BC and sent a form to recieve a copy now how can i fix this? is there a way i can give her a different last name? is it too late?

Default Birth Certificate and Paternity
If the father isn't named on the birth certificate you can probably file an affidavit of parentage with the father and get the birth certificate amended. Otherwise, you may have to go through a legal name change process. The County Clerk's office will probably be able to provide forms for either type of proceeding. When you go in, assuming you don't like either of the above options, ask if there is a way to amend a birth certificate and see if it would be permitted in your situation.

Sorry, but why would you regret giving your child your last name? My daughter had my last name for a few months, then had it changed to her father's last name. we are no longer together and now i wished i didn't change her last name. so think real hard before you do something like that. In order for me to change her name again, I would need his permission and would have to go through the courts to get her last name changed again.
