Changing My Childs Last Name To Current Husbands Last Name

My previous husband passed away. I am remarried and pregnant and I have a child from my first marriage. I want to change my first childs last name to the last name of my current husband. How do i do this? Do I have to go through a formal adoption process? Please help. I live in Ontario Canada.
