Default How to file an affidavit of parentage
I have 2 kids by this guy ,one of my kids have his last name but my other kid which is 1 years old doesnt ,and the reason being is when I gave birth he wasnt there cause he was incarcerated ,how can i make him sign a affidavit of parentage can i go to the hospital of where i gave birth or waht ,he says he's been busy working ,but I 'm sure he can take the time to do thi s I also want to get her name change to his last name he also wanted that ,but before i didnt i was acting all nasty,but can i go to the child support office and have him sign a affidavit of parentage or waht and i also want him to pay child support and i want full custody of my children ,please tell me what i should ,but he does claim my second child to be his ,i guess that's good
