Cool Affidavit of Parentage
I became pregnant with my daughter in Ohio. I moved to Michigan before she was born because her father and I were having problems. I had her in Michigan. Her father wasn't named on the birth certificate nor was he able to sign a paternity affidavit. We are now getting back together and getting married. My questions are:
1. What state do I get the affifavit from; who do I call to get one?
2. How do we get his name on the birth certificate? The cost?
3. How can we change her last name on the birth certificate? The cost?

I should let you know she is 6 weeks old

She was born in Michigan, so the easiest thing to do would be to submit an affidavit of parentage in Michigan. Forms and instructions are here.
