    Default Affidavit Of Support For I-485 Self-Petition

    hi everyone! my self petition I-360 got approved a week back and i got a mail asking me to file I-485 for adjustment of status. there are few questions that i have regarding the application.
    i have been married for 10 yrs and have been living with my husband here in US from the past 5 yrs. i have been through lot of abuse. my husband has withdrawn all his support and i am living with friends now. for I-485 they asked for affidivit of support. since my husband is the only person i know in this country and as i dont have any support, do self petitioners require to file affidavit of support?

    - i live in new mexico so should i file with the local INS or with the chicago office? the local INS website doesnt have form I- 485 on their list of forms that are accepted in albuquerque office. so where should i file it now?

    - do i have to get the medical examination before sending the form and send it along with the application or send the application and wait for them to ask me for a medical examination? i called the NCS number they said that i should send the application first and then they will send me a form to get filled and submitted to the office. but the instructions in I-485 says that i should include the medical examination form along with the application. so what is the right procedure??

    - this might sound dumb but i've got no clue.. i came to this country on v-1 visa. they asked for my non immigrant visa number in the I-485 form. there is no number saying its the visa number. i have two numbers on it - 1) the control number
    2) a number in the middle right hand coner in red with out any description. so friends with v-1 visa can you tell me which one is the visa number in it??

    I believe the instructions to the I-485 form cover your first two questions.
    Quote Quoting General.
    If you are filing your adjustment of status application with a USCIS service center, include your medical examination report with the application, unless you are a refugee.
    Quote Quoting Affidavit of Support.
    Submit an Affidavit of Support (Form I-864) if your adjustment of status application is based on your entry as a fiancé(e), a relative visa petition (Form I-130) filed by your relative, or an employment based visa petition (Form I-140) related to a business that is five percent or more owned by your family.
    Your immigration is based on an I-360, not an I-130.

    The red number is your visa number.
