Affidavit in Support of Motion for Parenting Time Assistance

I live in MN. I am filling out the forms for Parenting Time Assistance. I was wondering if one of your lawyers would be willing to look over my affidavits and tell me what they think? I can either post them here or message them to someone. I don't know if I included to much information, if I have irrelevant stuff in there, etc. I just want to get all my bases covered.
