    Default Civil Action in a U.S. District Court, Idaho

    I have a SS case where I have gone through the hearing process and my client was denied. I requested a review by the Appeals Council and it was denied. I would now like to file a civil action in the US District Court. Does anyone have forms or a step-by-step procedure that they use and would be willing to share or direct me to?

    Here's a sample pro se complaint form. It's drafted for New York, but can be modified.
