Default Who Picks A Baby's Last Name in Louisiana
I'm having a baby and the father and I are not together, I want him on the birth certificate but do not want the baby to have his last name. I want her to have my last name. In Louisiana if the father signs the birth certificate does the baby have to have his last name??

The real question is whether he will sign an application for a birth certificate if the child is going to have a different last name. Will he?

Yes he will. I have already talked to him about that.

Then it shouldn't be a problem. I can't find an official answer online, but you should be able to confirm that with the Department of Vital Records.
