    Question How To Request a Medical Impairment Rating

    Hi all. New to the forum and have a question about obtaining my medical impairment rating. I'm from New Hampshire.

    I was in a rearend collision 2 years ago and sustained back/neck injuries that I treated for about 1.5 years. My physiatrist in his documentation and in our verbal discussions states that I have likely reached "maximum medical recovery" and might need an impairment rating in the future. We discussed that I would likely have a permanent 10% disability total for neck and back, but it is not spelled out in his notes. I asked him if he would put that in writing and he said generally how it works is that the attorney sends him the request for an impairment rating with specific questions and he provides the information. I am not working with an attorney and so far don't have a need to (already in negotiations that are going pretty well), but I would like my impairment rating in writing should negotiations stall out.

    So, does anyone have a form/format for requesting a MIR, OR a link to a site that has a form or the format for requesting a MIR, OR information about what I should put in writing to obtain my rating from the doctor. I need to send him something formal to request it, I guess. Of all of this things I had to research and do to prepare for this, I didn't expect this part to be so difficult when he clearly states I've reached MMR and would probably need a MIR.

    Any help would be appreciated!

    I am not too familiar with the rating process but it sounds to me like the Dr. would request a reasonable fee for his time required to fill out such a form. When there is generally information to be shared from Dr to attorney, by said attorney, it generally carries a fee.

    It could be that it is not customary for a patient to ask for such a rating. Be up front with your Dr. and I believe that he will provide you with answers to the more specified questions. Take care and God Bless.
