How to fix a mistake in my taxes

I have filed 2005 taxes and my accountant made a mistake and forgot to include my income from my bussiness.
What do I do now? I would like to correct it. What form do I need to fill out? And where can I get this form?
(I can not go to that accountant, he is out of the picture now)

How do I feel it out?
Can I fill this form in in the computer? Or do I have to print it and fill out using a pen?
I was trying to save this form to Word but it would not save in the right way...

I'm not going to download it to check; but most IRS PDF forms can be filled on your computer. A caveat - unless you have a full version of Acrobat (not just the free Acrobat Reader) you can't normally save the information you type in. Instructions are usually provided with the form; but if you search the IRS site you can find pretty much any available instruction materials if you need more than is provided with th eform. You can also order forms and instructions through that site, and they will be mailed to you.
