How to Get Guardianship or Custody of my Girlfriend

Okay. My girl friend is 17 yrs old, I am 21, and She doesnt live with her parents. She wants to live with me and attend the school zone for my house. Her father is willing to sign over to me but the problem is we don't know where to go and what forms to file out. We have gone/called the clerk of court, juvinelle court and some others and they tell us that we need an attorney to file it for us or they give us the run around, we want to spend the least bit of cash to do this. We are just hoping there is a simple form to sign and appear infront of a judge and be done with it... Please if anyone has a clue or knows exactly what to do, it would be a god sent.

Her mother is actually dead. And her father has custody.
