My reference (state health ins. facts org.) only says: Balance Billing Laws Applicable in Oklahoma

Any person who knowingly and with the intent to injure, defraud or deceive any insurer, makes any claim for the proceeds of an insurance policy containing any false, incomplete or misleading information is guilty of a felony. Contact the Oklahoma Dept. of Insurance if you suspect fraud.
