Default Birth Certificate Ohio, Paternity Established in Pennsylvania

My question involves paternity law for the State of: Ohio

My son is an adult. When he was born, his father was not listed on his birth certificate. There was never a paternity issue, I was young and silly and didnt put his father's name on his birth certificate. I know it bothers him, but he doesnt discuss it. I do have verification that the State of Pennsylvania named his father as being his father. So I would like to put his father's name on his birth certificate. I was told I need a Paternity Affidavit. Is this correct?

    Thank you,

    Master Jhedi

That's exactly correct. In order to subsequently modify a birth certificate to add a different parent, you're required to show proof that the parent being add is actually the biological parent. A paternity affidavit would be sufficient proof of this. After that, you just need to fill out the proper government forms and you should be good to go.

. It sounds like you have a determination of paternity. That is adequate to have his name added to the BC. An affidavit of paternity would work as well if you and the father had filled one out. If you didn't, the determination of paternity will serve the same purpose.
I am not an attorney and any advice is not to be construed as legal advice. You might even want to ignore my advice. Actually, there are plenty of real attorneys that you might want to ignore as well.
