Default Waiting for Trial by Declaration Form

my ticket is from the michael d antelope courthouse in california. i received my court papers and tried to do trial by declaration. i had sent a check with the full bail amount and a letter requesting trial by declaration. it has been over a month since 5/6/2012 since i have sent in my check, and today is currently 6/13/2012 over a month and i still have not received my trial by declaration form. is there anything i can do to get the form without having to drive all the way out there to the courthouse since i do live more then 150 miles away. someone please help.

Sure there is... Here is form TR-200 Instructions to Defendant, and here is form TR-205 Request for Trial by Written Declaration which you complete with your information as well as the case information and include your declaration and any evidence.

The problem is, the forms by themselves, without knowing if your case has been set up in the court system as a TBD, and/or without knowing what the due date for you to submit your declaration, I m not sure how good the blank forms will do for you.

So it sounds like your only alternative is to drive that 150 miles!

    You broke it!!!!

You do get a one time link replacement with a new membership. Most would not want to use it this early on... You have no choice. here you go:

form TR-205 Request for Trial by Written Declaration
