    Default Adding Fathers Name to Birth Certificate

    My question involves name change laws in the State of: Pennslyviana
    My son is four years old and when he was born he dad didn't sign the birth certificate so as of now it says father unknown. Paternity has been established we have a custody and chils support order . The child has my last name but I would like to have the fathers name on the birth certificate. What is the process to do that?
