How Do You Change Your Child's Last Name in Kentucky

My question involves name change laws in the State of: Kentucky

I'm trying to find out what all I need to do to change the last name of my older son. He still has my maiden name and we are wanting to change it to his dad's last name. He is also my Husband. We also have 2 other boys and they have the same last name of my husband/their dad.

Does it cost to do this? Do you need a lawyer? Is there forms on the internet that I can print off? I live in the city of Louisville. County=Jefferson. If that matters.

Any help would be greatly appreciated.

If the father has been established (is on the birth certificate or subsequently affirmed) and both parents make the petition, it should be a routine approval.
Filing fee seems to be $43. You probably do not need a lawyer. After you fill out the form, call the clerk in your district county court and set up a time to submit the petition.

Although, if one parent ONLY is making the request you will need to authorize a parental consent form which will have to be sent to the other parent via certified mail (return receipt)
