    Default Adding father's name

    i wanted to know how do i go about adding my daughters fathers last to her name. she currently has just my last name but i want to add his name to the end but i don`t know what i have to do to do that..We live in Washington state

    I take it you were never married to the father. What is his position on this? Is he identified as her father on her birth certificate? Has he ever been formally recognized as her father, whether by affidavit of parentage or through a court determination of paternity?

    Just a question. Could a simple name change be done with the fathers last name being added to the moms last name with a hyphen? My daughters name is like this, but it was done when they asked for the spelling of her name for the birth certificate in the hospital.

    No we were never married and i just want to add his last name after my last name so i want to hyphen it...i did put his name down on papers when i filled them out for birth certi... but they didn`t put his name on it.....

    You could go to the court house, I believe it is there, to do a simple name change. It doesn't hold legal meaning, but meaning for the child who would be able to have it. How old is your child?

    She just turned 1 year on aug. 8th

    Call your cities court house, and ask what the procedures are to file for a name change on a 1 year old, and the cost to do so. Let them know you just want to add to the existing last name, not delete it.
