Medical Power of Attorney for an Eighteen-Year-Old

My question involves a power of attorney in the state of: OHIO

My daughter recently turned 18. She is a full body Reflex Neurovascular Dystrophy (RND) kid with Dysautonomia. With all that is going on with her it is overwhelming for her to handle all of her medical decisions and care. Her doctors have suggested I look into a poa or something giving me the authority to keep doing what I have been doing for her and with them regarding her care. I have talked to the Probate Court and they don't deal with medical poa's but do guardianships BUT she is not incompetent.
Any suggestions on what I can do and how? I don't know if this has any bearing but she does receive SSI.
