    Default Last Name Change Procedures in Illinois

    My daughter is 16 months. She cause her fathers last name. The father pays court issued child support. He has not visitation. How do I go about changing her last name to mine? Does the father have to be involved? Does it cost money?

    If you don't have the father's permission you will have to get a court order for the name change, and would typically have to convince the court that the name change is in the child's best interest. There will be a filing fee, and of course if you use a lawyer there would be lawyer fee.
