    Default How to Change Baby's Last Name to Mother's

    My question involves paternity law for the State of: NewJersey
    My daughter is 13 months & was born in a hospital at Newyork, while we were living in Connecticut at the time of her Birth. We had given her father's last name
    for her at her birth, same is there in her passport & SSN. Now we want to change it to her Mother's last name.
    We have shifted from New York To New Jersey. Also The daughter is American citizen but we are not. The father is in consent to the name change.
    How do we Proceed?

    File in the local court in the county in which you live for a Name Change for a Minor.

    You can get the forms from the Clerk of Courts.
