I had this shoved in my face in SnoCo Everett a few months back. I argued that "I honestly cannot see how a fill-in-the-blank affidavit can stand IN LIEU of an officer’s personal recollections of an event. Especially when every single one of the blanks that was filled-in or circled is taken directly from the Notice of Infraction itself." The prosecutor pretty much worded the exact same thing that you just said. Are you also working for the dark side of the force, Barry? hahaha.

Actually, he says it was checked "before and after each duty shift" but he never says if it was checked on this particular one. A long shot but maybe worth the argument.
