Is this homework? We don't do homework. What sort of POA are we talking about. I know of no state that has a general durable power of attorney "form." A few have ones specific to things like vehicle registration work and tax prep (as do the feds for the latter).
This is not hw. Working on a reference sheet of POA's for work. I am looking for which states have statutory POA forms in regards to financial decisions, similar to what the state of NY has. http://www.rocklandcountyclerk.com/clerkforms/poa.pdf
