Statutory Power of Attorney Forms

My question involves a power of attorney in the state of all states.

Which states have a statutory POA form for financial purposes, not limited to tax procedures?

Is this homework? We don't do homework. What sort of POA are we talking about. I know of no state that has a general durable power of attorney "form." A few have ones specific to things like vehicle registration work and tax prep (as do the feds for the latter).
