Default Misdemeanor and Reapplying for a F1 Student Visa

One year ago I went to court for not prof of insurance, I paid the fine. Three moths ago
an officer stop me for driving under suspension. ( I did not that my driver license was suspended). I went to court and I paid the fine. I`m going to go back to my country for the summer and I need to reapply for a visa F1 because I`m going to transfer to another college.
    1. When I`ll fill out the application, Do I need to check the box of crimes and offenses?
