Traffic Violation Ticket

My question involves a traffic accident in the State of: CA
I was involved in an accident and i was at fault. I got the courtesy notice from court but not sure what exactly I have to do here. There are 4 check boxes in the form with different amount listed next to each check box.
1. Proof of correction
2. To pay your ticket (bail forfeiture)
3. Request court appearance
4. Traffic School
Please advice what I need to do. Do I need to go to traffic school or appear for the court.
