Judgment for Temporary Visitation Order

My question is for the State of Oregon.
Would anyone happen to have an example of a judgment for a tempoary order or know where to find how to write one? I filed a petition for custody/visitation time. I am the non-custodial parent looking for a temporary order just for this summer while the petition is pending due to mediation and other necesssities. However the family law clerk told me I need to write a judgment for the Judge to sign in order to omit having a hearing. I'm running out of time and have not been able to find any information that would be helpful so far. Any and all help will be greatly appreciated.
